WHAT is a DOT Physical?

A DOT physical is a medical exam required by the Department of Transportation (DOT) to ensure commercial drivers are healthy and fit to safely operate a vehicle. The exam checks your vision, hearing, blood pressure, and overall physical health to meet federal safety standards. Once completed, you’ll receive a DOT medical certificate valid for up to two years.

WHY do I need a DOT physical?

A DOT physical is required to make sure commercial drivers are healthy, alert, and safe on the road. It helps protect both the driver and the public by checking for medical conditions that could affect driving ability. Passing your DOT physical keeps you compliant with federal regulations and allows you to maintain your CDL certification.

Safety

Driving large vehicles for long hours requires physical and mental endurance. The exam screens for conditions that could impair:

  • Vision or hearing
  • Alertness (e.g., sleep disorders)
  • Heart health
  • Mobility and coordination
  • Drug or alcohol use

Regulation Compliance

The Federal Motor Carrier Safety Administration (FMCSA) requires all CMV drivers in interstate commerce to pass a DOT physical and carry a valid medical certificate.

Public Responsibility

As a commercial driver, you’re responsible for transporting people, goods, or hazardous materials—often across long distances. A DOT exam confirms you can do so safely and responsibly.

Job Requirement

Many employers and state licensing agencies won’t hire or certify drivers without a current DOT physical on file.

WHO needs a DOT Physical?

A DOT physical is required for anyone who operates a commercial motor vehicle (CMV). This includes truck drivers, bus drivers, and delivery drivers who transport passengers or cargo across state lines. The exam ensures each driver meets the Federal Motor Carrier Safety Administration (FMCSA) health standards to safely operate a commercial vehicle.

If you hold or are applying for a Commercial Driver’s License (CDL), you must have a valid DOT medical certificate.

WHEN do I need a DOT Physical?

✅ Applying for or renewing a Commercial Driver’s License (CDL)

A valid DOT medical certificate is required before you can get or retain your CDL.

✅ Your current DOT medical certificate is expiring

DOT medical cards are typically valid for up to 24 months, but most health conditions (like high blood pressure or diabetes) require more frequent evaluations.

✅ Returning to duty after medical leave or a disqualifying condition

If you've been off work due to a medical issue, a new DOT exam may be needed before you can return to safety-sensitive duties.
