Developing a strong work ethic requires discipline, and maintaining good health plays a vital role in this journey.


Sports, too, play a powerful role in shaping individuals. Beyond physical fitness and disease prevention, sports help develop character traits such as discipline, responsibility, self-confidence, sacrifice, and accountability. These qualities are essential for becoming productive, engaged members of society.


We also recognize the importance of careers in transportation, particularly those requiring a Commercial Driver’s License (CDL). CDL holders are the backbone of our economy, transporting everything from essential goods to hazardous materials—and most importantly, the precious cargo of our children.

Hello, my name is Taylor! As a provider, I recognize that the medical world is intimidating to many. My mission is to simplify that experience for my patients. I strive to create a compassionate and approachable atmosphere, ensuring that you feel comfortable and understood throughout your healthcare journey. As someone who deeply understands the fears and uncertainties that can come with medical situations, my goal is to be a compassionate guide and support for you every step of the way.

My journey into the healthcare field stemmed from a passion I have nurtured since childhood. As a Board-Certified Family Nurse Practitioner, I bring over 13 years of medical experience to Triple M Medical Services. Throughout my journey, I have gained valuable experience in both inpatient and outpatient settings with focuses in critical care, pulmonology, and urgent care specialties. Additionally, I have continued my educational journey beyond graduate school, earning additional medical certifications that reflect my unwavering commitment to ongoing professional development and excellence in the field.

Autonomous Practice Disclosure

Provider: Taylor Slagter, MSN, AAPRN, FNP-BC, CCRN, CME 

Licenses and Certifications:  

AAPRN / AANP, Board Certified Family Nurse Practitioner / ANCC, Board Certified Family Nurse Practitioner / NPI / DEA

DOT Certified Medical Examiner / Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) obtained through AHA / Critical Care Registered Nurse Certification (CCRN). Will provide license and certifications credentials upon request.

Autonomous Practice Disclosure:

I am practicing as a Florida licensed autonomous Advance Practice Registered Nurse (APRN) and not as a physician.

As a Florida licensed autonomous APRN, I am authorized to provide primary care services without being under the direct supervision of a physician pursuant to Section 464.0123 of the Florida Statutes. I have completed at least 3 hours of continuing education in medical errors within the previous 2-year period. I have completed at least 3,000 clinical practice hours within the past 5 years under the supervision of a physician, including in primary care. I have not been subject to any disciplinary action under Section 456.072 or Section 464.018 of the Florida Statutes or any criminal prosecution in any jurisdiction within the past 5 years. I maintain professional liability coverage of at least $100,000 per claim with a minimum annual aggregate of at least $300,000 from an authorized insurer as required by Florida law for an autonomous practice.

Scope of Autonomous Practice:

As a board-certified Family Nurse Practitioner licensed for autonomous practice under Section 464.0123, Florida Statutes, I am committed to delivering comprehensive primary care services to my patients in Florida within my education and training. This includes: 

Health Promotion and Disease Prevention:

Conduct wellness physicals, sports physicals, and school physicals to promote health and ensure readiness for activities.

Provide patient education, counseling, and guidance on lifestyle modifications, vaccinations, and preventive screenings to maintain optimal health.

Assessment and Diagnosis:

Perform evaluations for acute conditions, including, but not limiting to, upper respiratory infections (URI), urinary tract infections (UTI), pneumonia (PNM), and bronchitis, as well as chronic conditions such as hypertension and diabetes.

Order and interpret diagnostic tests, including urinalysis, influenza, RSV, COVID-19, hemoglobin and hematocrit (H&H), glucose, rapid strep, annual labs, and general lab tests for health maintenance. Order send-out labs for STD screenings and other tests requiring further investigation through CLIA-certified commercial laboratories. Order imaging (e.g., X-rays, CT scans) for acute conditions such as fractures or abdominal pain to support accurate diagnoses.

Treatment and Management:

 Provide acute sick visits to diagnose and treat conditions such as URI, UTI, PNM, and bronchitis using evidence-based pharmacological and non-pharmacological therapies. Prescribe medications, including controlled substances (with limitations on psychiatric medications), to manage acute and chronic illnesses. Manage stable chronic conditions (e.g., diabetes, hypertension, asthma, COPD) using applicable guidelines.

Minor Procedures:

Perform subcutaneous procedures, including suturing minor lacerations, subungual hematoma release, and foreign body removal from skin or soft tissue.

Preventive Services:

Administer and interpret Purified Protein Derivative (PPD) tests for tuberculosis screening, order follow-up diagnostics (e.g., chest X-rays), and refer patients with positive results to specialists. Order and administer vaccines and immunizations, including influenza, MMR, Hepatitis A, Hepatitis B series, Tdap, and Varicella, following CDC schedules, providing patient education, in accordance with applicable Florida regulations (e.g., Florida SHOTS registry).

Behavioral and Mental Health:

Diagnose and treat uncomplicated mental health conditions, such as mild anxiety or depression, within the primary care scope, including prescribing antidepressants and providing counseling. This excludes any specialized psychiatric care, including psychotherapy or prescribing controlled substances for psychiatric conditions beyond a seven-day supply.

Referrals and Coordination:

Refer patients to specialists (e.g., infectious disease specialists, orthopedists) or emergency services, when necessary, such as for positive TB tests or complex fractures. Collaborate with healthcare providers to ensure continuity of care for complex conditions.

Facility-Based Care:

Admit, manage, and discharge patients from healthcare facilities for primary care-related conditions, such as acute exacerbations of chronic diseases, unless prohibited by federal law.

Certifications and Signatures:

 Provide signatures, certifications, and verifications for medical pre-op clearance, Department of Transportation (DOT) medical examinations, and sports and school physicals, and other legally required endorsements within my scope of primary care and education and training (excluding medical marijuana certifications).

Limitations:

My autonomous practice is limited to primary care services as defined by Florida law. Specialty services, such as managing lung nodules or performing bronchoscopies, require a supervisory protocol with a physician unless conducted in a primary care context. Cosmetic procedures (e.g., Botox for aesthetic purposes) are excluded.

As a patient under my care, you have the right to: 

  • Receive information about my qualifications and scope of primary care services practice.
  • Ask questions about your treatment and receive clear information about your diagnosis, planned course of treatment and options, risks, and prognosis. 
  • Seek a second opinion from another qualified healthcare provider.
  • Request a referral to a physician or other healthcare provider.
  • Participate in all healthcare decisions.
  • Refuse any recommended treatment or procedure.
  • Know that my office does not accept any type of insurance, including Medicare or Medicaid, and all primary care services are cash-based only. 
  • Know that you are free to choose, at any time, another healthcare provider who accepts your health insurance (if any) for the primary care services offered within my scope of practice.
