Prior to your DOT Medical Exam, you must complete the required form.

  3. Print the PDF form
  4. Fill in all of the BLUE fields(The green fields are for your provider.)
  5. Bring thecompleted form with you to your appointment


Please note: You are required to bring your driver's license with you to your exam. Please ensure that the address on your license is up to date and matches that which is provided on the form.

Prior to your appointment, please print and complete the required form.

  3. Fill out the patient required information on pages 1 and 2 prior to your appointment.
  4. Bring all FIVE pages of the physical exam form with you to your appointment.


School Entry Health Exam - Florida Health
